User documentation for FIELD VISIT & REPORT MODULE

1. Module access	Permission package for official control configuration: FVR_CONFIGURATION_PCK. Permission package for official control registration: FVR_REGISTRATION_PCK. There are two approaches to this module: 1. Configuration of official control This part of the module can be accessed by entering the admin console. In the left side menu click “OFFICIAL CONTROLS”. 2. Registration of official controls To register a previously configured control form, you need to enter HOLDING, HOLDING RESPONSIBLE, ANIMALS or ACTIVE PETS. In each previews, the official controls can be found in the top menu.
2. Create yes/no control form
	2. Create yes/no control form Once you enter the OFFICIAL CONTROLS section from the admin console, select “Create yes/no control form”.
	2.1) Create yes/nool form Title – Official control title Language – Control form language Scope – Where the control form should be previewed and registered (generally) Sub scope – Exactly specified type where the control form should be previewed and registered Will be answered multiple times – Check if question will be answered v. multiple times Question N* - Question content Score – Question score Correct answer – Which is the correct answer for the question Mandatory – Check if question is mandatory

3. Create control form (all question types)
	3.1) Create control form (all question types) In the official controls section from the admin console, select “New control form (all question types) Title – Official control title Language – Control form language Scope – Where the control form should be previewed and registered Will be answered multiple times – Check if question will be answered multiple times Question N* - Question content Score – Question score Sub scope - Exactly specified type where the control form should be previewed and registered Will have a long answer – Refers to long type of answer Will have a short answer – Refers to a short type of answer Mandatory – Check if question is mandatory Has multiple possible answers – Refers to multiple possible answers Answer option* – Answer option content

4. Preview of all official controls
	4.1) Preview of all official controls In the official controls section from the admin console, select “Official controls”.

7. Registration of yes/no control form
	7.1) Official controls in HOLDING After selecting one of the holdings from the holding list, click the “Official controls” button from the left side menu (if it’s animal the button will be at the top of the page).

	7.4) View control’s form history After the control form registration, if you click on the view history button from the right side, control’s form scoring and status (DRAFT) can be viewed.

	7.6) Lock control form On this screen if you click on the DRAFT, control form details will be shown where you can change and save your answers. The status will remain DRAFT until you lock the control form when the status will change to LOCKED. Same user can’t have more then one DRAFT control form.

	7.8) Unlock control form When control forms status is LOCKED and we click on it, we can still see controls details but we are no longer able to edit it’s data. Then unlock button is shown, but only specific type of user can unlock it.
8. Registration of control form (all question types)
	8.1) Official controls in ANIMAL After selecting one of the animals from the animal list, click the “Official controls” button from the animal screen at the top of the page (if it’s holding the button will be on the left side menu).

	8.4) Official control’s scoring and status After the control form registration, if you click on the view history button from the right side, control’s form scoring and status (DRAFT) can be viewed.
	8.5) Locked control form On this screen if you click on the DRAFT, control form details will be shown where you can change and save your answers. The status will remain DRAFT until you lock the control form when the status will change to LOCKED. Same user can’t have more then one DRAFT control form.
	8.6) Unlock control form When control forms status is LOCKED and we click on it, we can still see controls details but we are no longer able to edit it’s data. Then unlock button is shown, but only specific type of user can unlock it.
9. Delete control form

10. Edit question in an existing control form

	10.4) Edited official control’s form After successful edition of question, click on the edited control form to check the new version of question/s in the official controls.
