* Please enter the 11-digit personal number of the individual in the “Person ID NO*” field. In case of a legal entity, first check the legal entity button, and then enter the 9-digit identification number in the “Person ID NO*” field.
* You can’t fill in the “Recipient email”, because the system has a fixed email address to which the uploaded documents will be sent.
* By clicking the “choose file” in the “attachment” field, you will upload the user’s consent to access the personal information.
